Assessment
and design Our assessment is free for both individuals and organizations to
ensure that we can make the best decisions about their needs. As part of this
process, we take into consideration how someone lives now as well as what they
may need in the future to make sure that they are able to live comfortably with
the least number of limitations possible. We also want them to be able to feel
proud and confident in their surroundings so we focus on making homes as
aesthetically pleasing as possible while ensuring that all practical aspects
are taken care of too. Once the assessment is complete, we will work with
clients on a budget plan which includes all elements needed for renovations or
construction so they know exactly what they will be paying for when the work
starts. - Transitioning out of home Support The transition out of home support
service assists people who have a long-term requirement for ongoing disability
accommodation but are ready to start transitioning back into the community. The
support provided assists with issues such as navigating day-to-day living
skills and developing life skills. A staff member will attend your existing
residence on an ongoing basis until you're ready to move out and then we'll set
up another house if necessary (this usually takes around 12 months). You can
continue living at home until you're ready to transition by utilizing Disabled Travel
Assistance Perth.
Supported
living service Supported living provides affordable housing for people who have
high levels of support needs but still wish to live independently in their own
home with minimal supervision from outside agencies.
